2. How long do you keep your CCTV images for? (I hope they have not been destroyed.)

From the documents that were released, it would appear that your feedback team is still dealing with Formal Complaints.

I hope that after having been kept waiting months for the correct procedure to be followed for submitting my Formal Complaint, your CCTV footage of the assault has not been "automatically erased".
