Response to this request is long overdue. By law, under all circumstances, Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust should have responded by now (details). You can complain by requesting an internal review.

The below information is requested for all your PFI Contracts
• The name of the Project Company
• The name of the Service Provider for each Service provided under the PFI Contract
• The Service Payment for the last two Contract Years
• Deductions and Service Failure Points, Deficiency Points or equivalent for the last two Contract Years. Split by Service.
• Date that each Service was last market tested and all documents pertaining to the output of said market test
• The result of customer satisfaction surveys for each service for the last two Contract Years

• Does the Trust have a Monitoring Team who monitor the performance of the PFI Contract?

• Provide following for persons responsible for contract and/or performance management of the PFI contract
o Number of persons per salary band
o The % of time spent in managing/monitoring the PFI contract
• The results from Quality Monitoring Audits for the present and last Contract Year

PFI Trust Representative
• Name of the Trust Representative
• Their Contact E-Mail Address
• Their Length of time in role
• Their Previous PFI Experience
• Their salary bands
• The names of other PFI Contracts they manage

PFI Performance Management
• Minutes from the monthly performance meetings between the Trust and Project Co/Service Providers between the period November 2017 and November 2019.
• What other regular meetings take place between the Trust and Project Co/Service Providers?
o Copies of minutes from the above meetings between for the present and last Contract Year
• The dates of Joint Liaison Committee Meetings, including extraordinary meetings and minutes of those meetings for the present and last Contract Year
• The nature and issues raised in conjunction with the Dispute Resolution Procedure in the period November 2017 to November 2019?
o Copies of documents provided to the Adjudicator
o Documents pertaining to the Adjudicators decision for each issue
o The results of the Adjudicators decision for each issue
• What issues have been settled before reaching the Dispute Resolution Procedure?
o What was the value of each settlement?
o Provide copies of all documents pertaining to each settlement
