recorded when the investigation report is complete and shared.

Well that says it all about the PHSO, doesn't it?

You don't know how many draft reports found not upheld are then subsequently upheld on appeal. Why don't you collect and record this information? If you really cared about the quality of your reviews you would do this. You have absolutely no idea how well your case handlers are doing their job because you do not quality control it. Let's say that a particular case handler always finds cases not upheld at draft report stage, then people appeal and most of them then receive a final report that is upheld. You don't know that you have a useless case handler, and more importantly the general public don't know.
You have now proved that the PHSO quality control is nothing but a seriously bad joke.

I asked for information concerning the 10 PHSO caseworkers who rejected (Not upheld) the most cases during 2014/15:

There was a big difference between these 10 and the corresponding 10 from the Scottish Ombudsman.

The PHSO's 10 did not uphold 396 complaints but upheld/partly upheld 145.

The Scottish Ombudsman's 10 did not uphold 245 complaints but upheld 200.
