Dear Malcolm,

FOI1406

Further to your FOI request below, please can you clarify what information
you require in relation to question 5?

You have asked us to confirm ‘How many serious/adverse incidents have been
reported related directly to the transportation of patients by taxi’.

·         Do you mean, out of the total number of serious / adverse
incidents reported during the last 12 months, how many patients were
conveyed by taxi?

·         How do you define ‘serious / adverse incidents’? We currently
have 4 categories relating to incidents 1) Category 1 (life threatening)
2) Category 2 (Emergency) 3) Category 3 (urgent) and 4) Category 4 (less
urgent). Please can you confirm which category you would like data for?

Please note that the deadline for response to question 5 will be put on
hold until we receive a response from you.

Many thanks

Malcolm Macgregor

Dear FOI Enquiries,

I would like the total number of adverse incidents (including category 1, 2, 3 and 4) which were directly related to, or caused by, taxi conveyance. This is as opposed to any adverse incidents during which the patient was coincidentally transported by taxi, but that was not related to or caused by the conveyance by taxi as such.

If you do not hold this information you may disregard question 5.

Yours sincerely,

Malcolm Macgregor

and physically suitable to travel in a taxi and there was no requirement
for clinical intervention or supervision.

I trust this information is of assistance and would ask that if you intend
to use the information in a future media article or broadcast, we are
given the opportunity to provide a qualifying comment.
