Dear Tom,

Thank you for your recent request for information.

After having a discussion with a member of the relevant team, we can confirm that our private providers do not routinely use paper patient report forms, they use the same electronic system as our Trust crews do.

Could you please confirm if you still require a copy of the paper PRF?
