P45 personal details incorrect

The request was refused by HM Revenue and Customs.

Dear HM Revenue and Customs,

If your ex employer provides you with a P45 that has part of your personal details incorrect:

1) How can the incorrect P45 be rectified.

2) Does it have to be rectified?

3) If so, who's responsible for rectifying the mistake?

4) Is it the responsibility of the Ex Employer to notify HMRC that they have sent them a Part 1 of a P45 with some incorrect details.

5) Can the Ex Employer issue another P45 to the ex employee with the correct details?

For clarity the national insurance number is correct in this scenario and is not the incorrect detail.
