Thank you for the information sent, However some the FOI questions are not completely answered.
Can i ask that you could amend :

[a] Consultant names are not given in the OHPCC worksheet – yet these names are given in the SWAH worksheet. The same data should be given in both. Can the consultant's name to be added to the OHPCC worksheet as it is in SWAH worksheet.

[b] There is a repeated misspelling of ‘OHPCC’ as ‘OPHCC’ in both Trust answer and the spreadsheet – this affects searches and collating data in particular.

Yours sincerely,

helen hamill

Dear Ms Hamill

I refer to your email and queries below regarding our Freedom of Information response (our ref.: FOI/19/150) which was issued to you on 31st May 2019. Regarding your queries, please see our Trust response below;

[a] Consultant names are not given in the OHPCC worksheet – yet these names are given in the SWAH worksheet. The same data should be given in both. Can the consultant's name to be added to the OHPCC worksheet as it is in SWAH worksheet.

Response: Regarding the consultant names, these have now been added to the attached spread-sheet.

[b] There is a repeated misspelling of ‘OHPCC’ as ‘OPHCC’ in both Trust answer and the spreadsheet – this affects searches and collating data in particular.

Response: Regarding the misspelling and column heading, a column heading has now been inserted ('Primary Base') on both tabs in the attached spread-sheet and the spelling has been rectified.

We hope you find this helpful.
