Dear Northern Health and Social Care Trust (Northern Ireland),
I am requesting information as per the following.
1) For all communications between the Trust and the Board regarding the removal of Out Of Hours services from Moneymore to Mid Ulster Hospital
2) A copy of the floor plan of the Out Patients department that shows where the Out Of Hours will be located.
3) Will any other current services be displaced, removed or stopped to accommodate the out of hours service
4) was the Minor Injury Unit considered as a location for the Out Of Hours, if so provide all documentation as to while it will not be located at the Minor Injury Unit in Mid Ulster
