Out of County Acute Bed placements
Dear Kent and Medway NHS and Social Care Partnership Trust,
I have read that In June 2016, Kent and Medway NHS and Social Care Partnership Trust (KMPT) had 76 patients in private beds and this figure included some patients in 'out of county' placements. In 2017 the Chief Executive of KMPT declared publicly that there were NO patients in private beds. I am also aware that the same Chief Executive of KMPT said in June 2017 , "Sometimes, if someone needs a particularly specialist service that is not available in Kent, we have to find the next best thing...".
My previous FOI request regarding the number of 'out of county' patients in 'specialist beds' was not fully answered because it was said "KMPT does not place patients into ‘specialist beds’. KMPT only use private beds for ‘acute beds' ". Therefore I will reword my request and use the term 'acute beds'.
So please could you answer the following:
1. When the term 'Out of County' is used does it mean
a) outside of Kent or
b) outside of Kent, Surrey & Sussex combined? or
c) any other definition
2. If other counties are included in the Kent figures for 'Out of County' what date were these counties first included ?
3. How many K&M NHS Mental Health patients were placed in 'out of county' private 'acute beds':
a. in 2016?
b. in 2017 ?
c. in 2018 ?
4. How many of those K&M NHS Mental Health patients who are currently in 'out of county' private 'acute beds' have been there :
a. longer than one year ?
b. longer than two years?
5. Have there been some K&M NHS Mental Health patients whose needs could not be met by local acute mental health services provided by KMPT in 2016-2018?
If yes, How many K&M NHS Mental Health patients had needs that could not be met by local acute mental health services provided by KMPT?
a. in 2016?
b. in 2017?
c. in 2018?
6. How many K&M NHS Mental Health patients had needs that could not be met by local acute mental health services provided by KMPT, were not placed in 'out of county' beds by KMPT but were instead referred to 'specialist services' through CCG's?
a. in 2016?
b. in 2017?
c. in 2018?
7. According to KMPT what is the difference between a 'specialist placement' and an 'acute bed'?
8. It has been stated that "If the patient’s needs can be met by local acute mental health services provided by KMPT then this would not be a specialist placement."
a. Is a 'specialist placement' based only on mental health specific needs?
b. Does this mean that the a 'specialist placement' is needed where the specific mental needs of this patient are beyond what KMPT have the appropriately skilled mental health medical resources to provide and/or a very specific type of therapy and treatment are to be given?
c. Has a 'specialist placement' ever been used in the situation where KMPT do not have general resources to provide care for the patient eg. insufficient bed spaces in local NHS mental health facilities etc ?

Following an investigation and search of our records I can confirm the following Kent and Medway NHS and Social Care Partnership Trust (KMPT) information:-
1. When the term 'Out of County' is used does it mean
a) outside of Kent or

Outside Kent is any area outside of Kent and Medway.
2. If other counties are included in the Kent figures for 'Out of County' what date were these counties first included ?
Other counties are not included

We do not have any patients currently in an Acute External Bed.

Yes – this would be any patient admitted to a private younger or older adult acute mental health bed.

Kent and Medway Social care and Partnership Trust are unable to answer this question as it would need to be requested directly from the CCGs as it is the CCG’s who receive the referrals.
7. According to KMPT what is the difference between a 'specialist placement' and an 'acute bed'?
Acute mental health beds are for service users experiencing a short term acute episode of mental ill health who require short term acute bio-psycho-social mental health interventions.
A specialist placement is for a service user with a specific and enduring complex care need that requires specific specialist interventions within a specific and bespoke setting. There are a range of ‘specialist care needs’ and specialist placements that Kent and Medway service users are referred to, funded by the CCGs. Examples include neuro-psychiatry placements, locked rehabilitation, services for people with complex presentations and difficulty in containing their behaviours due to significant risk to themselves or others and cannot be safely treated on a general acute in patient ward
8. It has been stated that "If the patient’s needs can be met by local acute mental health services provided by KMPT then this would not be a specialist placement."
a.Is a 'specialist placement' based only on mental health specific needs?
Specialist placements cater to a specific and specialist need.
b.Does this mean that the a 'specialist placement' is needed where the specific mental needs of this patient are beyond what KMPT have the appropriately skilled mental health medical resources to provide and/or a very specific type of therapy and treatment are to be given?
Across the health and social care spectrum there are a small proportion of service users whose health needs do require specific and specialist interventions which include specific types of therapy and treatment within a 24/7 bespoke setting, that KMPT may not be funded or commissioned to provide.
c.Has a 'specialist placement' ever been used in the situation where KMPT do not have general resources to provide care for the patient eg. insufficient bed spaces in local NHS mental health facilities etc ?
Kent and Medway Social care and Partnership Trust are unable to answer this question as it would need to be requested directly from the CCGs as it is the CCG’s who receive the referrals.
