Re: FOI 16/208

Dear Jesse,

Thank you for your recent request under the Freedom of Information Act 2000. The Trust has no definitive Deprivation of Liberty Safeguards (DoLS) structure, all managers (usually of in-patient units) will request/grant DoLS authorisations. In terms of governance the safeguarding team and the Mental Health Law Service will oversee the numbers of applications for reporting on contractual assurance reports. This structure is as follows:

Gail Briers, Chief Nurse/Director of Operational Clinical Services, [email address]
Steve Hull, Deputy Director of Nursing and Quality, [email address]
Lorna Griffiths, Head of Safeguarding, [email address]
Sarah Shaw, Named Professional Safeguarding Adults, [email address]
Ann Cunliffe, Mental Health Law Manager, [email address]

All managers can be contracted via the Trust switchboard on 01925 664000.

Review Procedure

We hope this provides the information required. However, if it is felt that the way the request was handled or the content of the response is unsatisfactory and should be reviewed, please write to the Information Governance & Clinical Coding Team at [5 Boroughs Partnership NHS Foundation Trust request email] requesting an internal review and stating the reference number at the top of this document.

A request for an internal review should be submitted within 40 working days of the day this response was sent. Any such request received after this time will only be considered at the discretion of the Head of IT Systems and Services.

If it is then felt that the outcome of the internal review is unsatisfactory, a complaint can be made directly to the Information Commissioner’s Office (ICO) for a decision, but it should be noted that the ICO will not make a decision unless the Trust’s internal review process has been followed first.
