Please can you provide under the Freedom of Information Act a full management structure chart for the Trust inclusive of all 'Heads of Service,' 'Assistant Directors' and Directors with the full job titles and the names of individuals who hold these positions.

Thank you for your request for information regarding our structure which
we received on 27/8/19. We have dealt with this as a Freedom of
Information request and I attach a leaflet containing further information
about both your rights and our obligations under this legislation.
