I am contacting you further to your email of 2 March 2021.

 

Following a review of your email, it has become apparent that
clarification is required in relation to some of the noted questions;

 

Please can you provide an up-to-date Organisational Structure chart for
the Trust?

I am aware as to the one published on your web site for the Board of
Directors.

Would you provide one including the names and job titles for other senior
members of the trust such as the Deputies and any Site Leadership or
Divisional Leadership teams?

 

It would be necessary to clarify your definition of “senior members” of
the Trust.  As I hope you can appreciate the Trust has a duty to ensure
staff privacy is maintained at all times, and therefore we do not publish
the names of any staff who’s role is not public facing (*Exception of
Trust Board, Executive Directors, Divisional Clinical Directors, Division
General Managers)

 

It would therefore be necessary to clarify and provide examples of the
types of “Deputies and any Site Leadership or Divisional Leadership teams”
you refer to.

 

In addition, in order to formally process your FOI (Freedom of
Information) request on the systems used by the Trust, I would be grateful
if you could please confirm the full name of the person/s requesting this
data. 

 

*Please note that in light of the ongoing pandemic, all Freedom of
Information submissions at this time may be subject to delays due to

To clarify my request, I would be grateful to receive information on staff holding the position of Divisional Clinical Director, Divisional General Manager and, where possible, Deputies to any of the above or of any of the other Directors (e.g. Deputy Chief Nurse)

In regards to releasing my full name, ICO guidance states that, "Any variation of the requester’s title or first name combined with their surname (e.g. Mr Smith or John Smith) will be sufficient to meet this requirement. However, a first name or surname provided in isolation, or a set of initials, will not." I believe that what I have provided is sufficient to fulfil their conditions to be formally processed.

I would like to thank you for providing clarification in relation to your
Freedom of Information request.  In response to your noted advice:

 

In regards to releasing my full name, ICO guidance states that, "Any

To confirm, the request for the provision of a full name is owing to the
electronic systems used by the Trust to record Freedom of Information
(FOI) requests.  As I hope you can appreciate, there can be a number of
instances where any persons submitting an FOI request may have the same
surname as another individual.  In any instance where a person may wish to
contact the Trust again with reference to their original request, and they
may not readily have access to the provided FOI reference number, it can,
in some instances, be difficult to confidently identify the individual
concerned.

 

Please be assured that your wish to provide surname only is acknowledged,
and I would therefore please ask that you keep a note of the above
reference number should you have any questions/concerns.  Your FOI request
will of course now be processed accordingly.
