Online form for registering new domain names?

Hi,

Further to a previous FOI request [1] (and more specifically, the COI's response), in Minute 9 of the naming_and_approval_minutes_2007-09-26Q29525277.doc [2], I was wondering if it's possible for the following to be released --

(a) the URI to the form

(b) the source-code of the form/scripts that make the form function.

(c) the amount of applications, via the form,

(i) made, (ii) rejected, (iii) approved

-- and from how many entities (along with when the requests were made/decided).

Ideally (under (c)), it would be lovely to see who (by which I'd like the organization/authority/corporate body, not individual) made applications, what it was for (i.e., the fully-qualified domain name request), the status of each request, and when it was made/decided on.

Is that possible?
