I would be most grateful if you would provide me, under the Freedom of Information Act, details in respect to the contract below.
CCP079 - Provision of an Occupational Health Physician Service:

• What are the contractual performance KPI's for this contract?
• Suppliers who applied for inclusion on the contract and were successful & not successful at the PQQ & ITT stages
• Service utilisation volumes
• Actual spend on this contract, from the start of the contract to the current date
• Expected end date of the contract?
• Has there been any significant supplier performance issues in the delivery of this contract?
• Could you please provide a copy of the service/product specification given to all bidders for when this contract was last advertised?
• Can you outline the level of Social Value delivered by the supplier as part of this contract?
• Is there an extension clause in the contract and, if so, the duration of the extension?
• Has a decision been made yet on whether the contract is being either extended or renewed?
• Who is the senior officer (outside of procurement) responsible for this contract?

Freedom of Information Act 2000, if your request also relates to
Environmental Information e.g. information about the environment or land

Healthworks Ltd were the successful bidder.
• Service utilisation volumes Since October 21 ,we have used the OHP
service, 2 days per week so 16 appointments per week. They carry out
remote and face to face appointments in 3 clinics.
• Actual spend on this contract, from the start of the contract to the
current date - £164,385 from 1^st October 2021 – 12^th February 2023
• Expected end date of the contract? 30^th September 2023
• Has there been any significant supplier performance issues in the
delivery of this contract? No
• Could you please provide a copy of the service/product specification
given to all bidders for when this contract was last advertised? Attached
• Can you outline the level of Social Value delivered by the supplier as
part of this contract? Social Value did not form part of this project.
• Is there an extension clause in the contract and, if so, the duration of
the extension? Yes the initial contract period was for one year with an
option to extend for an additional one year which we have taken.
• Has a decision been made yet on whether the contract is being either
extended or renewed? Decision not made, part of an ongoing review of
service.
• Who is the senior officer (outside of procurement) responsible for this
