Dear The Co-operative Pharmacy,
I would like to request the following information from the Co-operative Group. How many of your employees have been referred to your occupational health provider (Atos) since you contracted them as your occupational health provider? Also, how many employees were declared as being unfit for work and in particular, how many employees have left the co-operative group after being declared unfit to continue working for you?

What date is the contract due to be renewed?

Dear Janice Perkins (Pharmacy Superintendent's Office),
I have just received your reply and am very disappointed that you have failed to read who the information was requested from. The FOI request was clearly addressed to the Co-operative Group. Whilst you state that you will not answer the request as you are the Co-operative pharmacy, I would appreciate it if you would ensure that the request is forwarded onto the department that deals with Freedom of Information requests at the Co-operative Group. As stipulated, I wish for this FOI request to be forwarded onto the Co-operative Group. They are not listed on this website (from what I can see) therefore, I would very much appreciate if you could forward the request onto them to allow a speedy response.
