clarification from you as follows:

In terms of collapse/cardiac arrest - please confirm whether you require
information pertaining to pregnancy, during labour or postpartum/puerperal
as each of these are recorded separately.

For ERPC, please advise whether the information is for miscarriages
(presumed) or whether after delivery (retained placenta, also classed as a
product of conception.

If you are able to provide relevant clarification to these points, this
will enable us to respond in full to you.

I am writing to request an internal review of University Hospitals
Coventry and Warwickshire NHS Trusts handling of my FOI request Obstetric
data for last 5 years.

I have chased this data and clarifications, please now provide the full
response. The last response indicated another 20 day wait. This is
incorrect and not in line with the FOIA.

Your original request was received electronically on the 28th March 2023
through our Freedom of Information management system, Infreemation. 

Following a review of your request we identified the relevant staff and
these were then contacted and chased for response.

A clarification request was made by a staff member on the 27th April 2023
to which we then emailed yourself on the same day seeking clarity. 
Clarification from yourself was received on the same day and relayed to
the relevant department.

Unfortunately the relevant person who could provide the information no
longer works at the Trust  and due to other staffing issues, they were
struggling to provide responses to the unanswered questions in your
request. 

The responses were finally provided and disclosure made to you on the 13th
July 2023.

We are extremely sorry for the delayed response to your request which did
exceed our usual time frame for responses to Freedom of Information
requests; for this we do apologise.  

As we have provided the information that we do hold your request is now
closed.  We trust that this is satisfactory but if you are dissatisfied
