Oaths made in court and perjury

I understand that taking the oath is recorded in Crown Courts. Please could you let me know the following information:

1) Of those lawfully sworn in a judicial proceeding as a witness or interpreter, the percentage of oaths taken on a holy book and the percentage of affirmations (not using a holy book).

2) Of those lawfully sworn in a judicial proceeding as a witness or interpreter and subsequently charged with perjury, the percentage of oaths taken on a holy book and the percentage of affirmations (not using a holy book)
