Numbers of vaccinated and unvaccinated staff

Please can I have the following information, presented in spreadsheet form:

Please break down staff numbers in appropriate categories, for example: front facing, medical, office, cleaning, etc.

0. Total number of staff.
1. Number of staff with three (or more) doses of a Covid-19 vaccine.
1a. Number of staff who have booked a third dose of the Covid-19 vaccine but have not yet received it.
2. Number of staff with two doses of a Covid-19 vaccine, but not a second one.
2b. Number of staff who have booked a second dose of the Covid-19 vaccine but have not yet received it.
3. Number of staff with first dose of a Covid-19 vaccine, but not a second one.
3b. Number of staff who have booked a first dose of the Covid-19 vaccine but have not yet received it.
4. Number of staff who cannot have a Covid-19 vaccine for a health reason.
5. Number of staff who cannot have a Covid-19 vaccine for other reasons.
5a. An anonymized list of these reasons.
6. Number of staff expected to be terminated under the mandatory vaccine requirement.
7. In appropriate file formats (such as PDF), details of plans/contingencies to cover/replace these staff.

If any of these points would cause the whole request to be rejected please consider the rest of the request separately to the original timeframe but inform me of the decision to reject at the time it is made.
