Number of events where emergency GLASS pre filled syringes do not connect to luer Devices.

Response to this request is long overdue. By law, under all circumstances, Highland NHS Board should have responded by now (details). You can complain by requesting an internal review.

Dear Highland NHS Board,

In 2011 there were MHRA alerts into incompatibility of glass pre filled syringes not connecting to the luer needle free systems that are used in many hospitals.

Could you look at the first 5 years 2011-2016 and report the number of recorded events or incidents that occurred when the glass pre filled syringes had issues connecting to the needle free device that it used in the trust.

the glass pre filled syringes are
Adrenaline
Amiodarone
Atropine

Could you then also report from 2016-present day for the same question.
