Dear Highways Agency,

Under the terms of the Freedom of Information Act I asked to be provided with the following;

The Number of Incidents on the M40 near J12 MP141/2 on the 5th October 2012.

Please can you advise what the reported circumstances of these incidents were.

please provide us with the Highways Incident Report forms for these incidents.

details of any damage caused as a result of these incidents and the costs relating to the damage.

please advise if Towergate Claims Managment have been Instructed to recover costs in any of thses incidents.

The Highways Agency is responsible for operating, maintaining and
improving the Strategic Road Network in England on behalf of the Secretary
of State for Transport.  This consists of the Motorway network and the
Major Trunk roads. A map of the roads for which we are responsible can be
