I write to request the following information from NHS South Manchester CCG:

1. How many Individual Funding Requests for homeopathy services have been made to the CCG since July 1st 2014?

2. How many of these Individual Funding Requests were successful, if any?

3. What is the total cost of the homeopathic treatments/services covered by the above successful Individual Funding Requests?
