Has proper authority and guidance for Nor West Medical Ltd, who are currently operating both Paramedic Emergency Service and Urgent Care Service support shifts, the correct authority to utilise the NWAS NHS Trust paper Patient Report Form (PRF)?
Are you aware that Nor West Ambulance Ltd have copied the standard NWAS paper PRF, including information bundled which are printed on the entire pack?
Have you received any complaints regarding misinformation and confusion from service users regarding Nor West’s PRFs?

Hi Matt,
In response to your Freedom of Information request regarding patient
report forms, please see the information below.
Has proper authority and guidance for Nor West Medical Ltd, who are
currently operating both Paramedic Emergency Service and Urgent Care
Service support shifts, the correct authority to utilise the NWAS NHS
Trust paper Patient Report Form (PRF)?
Within the contract we have with third party providers they are required
to have their own PRF and the specification of what needs to be on the
form is based on the NWAS prf.
Are you aware that Nor West Ambulance Ltd have copied the standard NWAS
paper PRF, including information bundled which are printed on the entire
pack?
In the interests of collaborative working with private providers and to
share good practice, providers have been able to use the NWAS PRF as the
basis for designing their own which Norwest have done as they know this
will meet our requirements.
Have you received any complaints regarding misinformation and confusion
from service users regarding Nor West’s PRFs?
We are not aware of any confusion or misinformation from service users
however, we would expect consistency in PRF design would reduce confusion
and misinformation. We have also had requests for PRF’s from Norwest for
coroners reports without any concerns raised by them or hospitals.
