No of Deaths Caused by Negligence in the Hospital

Dear Lancashire Teaching Hospitals NHS Foundation Trust,

I will be most grateful if you will provide to the public the following facts:-

1) The total number of professional negligence claims received by your hospital over the last 6 year period. (listed by year)

2) The total amount of money paid out each year in professional negligence claims over the same 6 year period.

3) The total number of deaths which have occurred in the hospital each year during the past 6 years

4) The total number of admissions per year over the past 6 years.

5) The Annual Budget allocated to your trust each year over the past 6 years

6) The Annual Budget allocated to the Royal Preston Hospital each year over the past 6 years.

7) Why millions of pounds were wasted in equipping an Accident and Emergency Department in Chorley Hospital, when the unit has never been opened or operational. Please name the personal responsible for making this expenditure.

8) Does the trust operate a Euthanasia policy for patients over the age of 75 years? Either through negligence or neglect?

I will be most grateful if you will provide this information for each and every hospital managed by this trust including the Royal Preston Hospital.

Dear Freedom of Information (LTHTR),

Thank you for your distinction, as doctors are also "professionals" being members of a professional body governed by the "General Medical Council"[ http://www.gmc-uk.org/about/role.asp].

However, as you have also highlighted Clinical Negligence we can use your description in addition to the above, to obtain complaint information which has been alleged against ALL other hospital staff e.g. administrative, nursing and care-teams, as in my example of a hospital pharmacist neglecting to change the drip of a patient from sodium to potassium.

The information I seek is how many claims your hospital has received from patients/relatives either directly or via their solicitors. (which counts as one complaint), during the relevant 12 month periods, between 1st January of the relevant year to the 31st December.

I also seek basic information as to the number of contractors hired (doctors and nurses) by the hospital (Royal Preston specifically) prior to and during times of formal inspections by any outside bodies across the years 2015 and 2016. I am looking for correlations between the number of "staff" during and prior and after such visits.
