Dear Sheffield Teaching Hospitals NHS Foundation Trust,

- The total number of in-patient deaths (including emergency department deaths)
- The number of deaths subject to case record review
- Of these deaths subjected to review, the number that were judged more likely than not to have been due to problems in care
If data is not available at the beginning of 2018, please supply data from the earliest point after when data was collected.
Yours faithfully,
Alex Clark

Dear FOI (SHEFFIELD TEACHING HOSPITALS NHS FOUNDATION TRUST),
Many thanks for your response. However, could you please address the following point:
I have requested the data up to the most recent month for which it has been collected, but the data in the attached response runs up to December 2021.
Would it be possible to receive the data for the first months of 2022 for which data has been collected? To May?
Yours sincerely,
Alex Clark
Dear Alex
Thank you for your email.
I have contacted the relevant department who have advised the following:
‘The data is up to and including December 2021 - I can confirm that we
routinely report quarterly, six months in arrears to ensure completion of
relevant SJRs and therefore more recent data would be incomplete.’
Kind regards
Joanne
