If data is not available at the beginning of 2018, please supply data from the earliest point after when data was collected.

Dear [email address],

Many thanks for your response. However, I am not fully satisfied with the information you have provided.

I am seeking Learning from Deaths data on a monthly basis, which is not available in the Learning from Deaths reports found in the board papers, as the figures there are broken down on a quarterly basis. I am also seeking the data accessible in the format of a single dataset, rather than spread out across multiple reports. I am also seeking the data up to the most recent month for which data is available.

If helpful, I would like to receive the data in the following format that could be provided as a spreadsheet:
- Three tables: one for total number of deaths, one for total number of deaths subject to case record reviews, and one for total number of deaths judged to be due to a problem in care
- The columns of each table are a month
- The rows of each table are a year
- The value in each cell is the corresponding value for that month and year, or blank data has yet to be collected for that time period

Yours sincerely,

Alex Clark

Please see attachment.  However, please note that we are unable to provide
the rest of the information you requested because of the limits of Section
12 of the Freedom of Information Act.  (We have been advised that each of
the clinical divisions would need to check each death versus mortality
review completed manually.  This is likely to take a minimum of 37.5 hours
for each division to complete, and would probably need to involve two
members of staff to locate reviews, cross-check etc.)
