IN RE: FREEDOM OF INFORMATION ACT 2000

I write to request you provide me with the following information, pursuant to the relevant provisions of the Freedom of Information Act 2000 ("the Act").

Please note, in respect of all of the below listed queries, I am seeking data as it stood on 1 October 2020.

1. What is the headcount total of NHS-funded patients which are registered with the practice?
2. Of that headcount total, please provide a numerical breakdown which confirms separately in respect of both your "digital" cohort of patients under the 'GP at Hand' brand, and your "physical" cohort of patients which have been subcontracted to another GMS-contracted practice within the NHS Hammersmith and Fulham CCG area, each of the following:
(a) How many of the headcount are: (I) male, (II) female, and (III) other/not specified;
(b) How many of the headcount fit within each of these age bands: (I) 0-4, (II) 4-14, (III) 15-17, (IV) 18-26, (V) 27-36, (VI) 37-46, (VII) 47-56, (VIII) 57-66, (IX) 67-76, (X) 77-86, (XI) 87 and above.
(c) How many have their place of residence located within the boundaries of NHS Hammersmith and Fulham CCG?
(c) For those whose place of residence IS NOT located within the boundaries of NHS Hammersmith and Fulham CCG, please provide a list of the CCG organisational area which is lived in, as well as the headcount number which live within that CCG area,
3. Within the preceding twelve months, how many registered patients have made complaints (by any communication medium) which have fallen to be dealt with under The Local Authority and National Health Service Complaints (England) Regulations 2009?
4. Of the total number of registered patients who have made such complaints, what is the total number of complaints which have been recorded by the practice?
5. Please provide a copy of all information you hold which relates to your policies, practices and procedures in respect of (I) complaint registration (II) complaint investigation and (III) complaint management - including particularly any "internal" information which you hold such as standard operating procedures, written policies, and complaints guidance; and whether produced internally by your organisation (or employees/contractors thereto), or sourced and/or supplied by any external person, party, body, publisher or organisation.
6. How many of the total number of complaints (as described at point 5) have been fully resolved as of 20 November 2020?
