I am writing to request information under the Freedom of Information Act 2000 regarding the Department of Health and Social Care’s contracts with Cygnet Health Care. Please provide the following:

1. A list of all contracts awarded to Cygnet Health Care since 2010, including start and end dates, scope of services, and total contract value. Copies of the contracts or detailed descriptions of the contractual obligations, particularly for mental health and CAMHS services.

2. The criteria and processes used to award contracts to Cygnet Health Care, including performance metrics or quality standards and how compliance with these criteria and standards is assessed before and during the contract period.

3. Procedures and actions taken in cases of contract breaches by Cygnet Health Care, including financial penalties, corrective action plans, and monitoring processes. A list of all Contract Performance Notices issued to Cygnet Health Care since 2010, including dates, nature of breaches, and required actions.

4. Copies of remedial action plans submitted by Cygnet Health Care in response to Contract Performance Notices, along with DHSC assessments and reviews. Processes and outcomes of any reviews or monitoring activities undertaken to assess compliance with remedial action plans.

5. Information on sanctions imposed for breaches, including financial penalties, service credits, or other measures. Details of any escalated enforcement actions, such as appointing independent experts or terminating contracts.
