Dear Amanda-Jane

Thank you for your information request.

Information Act. This is because the CCG is not responsible for the
issuing of NHS/ Hospital numbers or birth certificates and therefore does
not hold the information that you have requested.

However, I can provide some information and guidance which I hope will be
helpful to you.

NHS number - NHS Numbers are unique and given when you register for NHS
services, for example with a GP. Since 2002 all babies born in England
automatically receive an NHS number.  For more information please refer to

Hospital number - These are only generated when you attend hospital. The
number is unique to you in that hospital.

Birth certificates - A copy of your birth certificate should be available
at the registry office where your birth was registered.

We trust this provides you with the information you require.

Kind regards
