Neonatal deaths and FOIs
Dear Countess of Chester Hospital NHS Foundation Trust,
I would like to request the following information under the FOIA:
1) A listing in Word or Excel format from of all FOI requests since April 2016 for the following:
- Unique FOI ID
- Date of FOI request
- Description of request, as recorded on the FOI system
2) The number of monthly, since April 2012
- neonatal and stillbirth deaths
- births
3) The RCPCH November 2016 service review report, on page 7 / 13 under paragraph 3.7, states that “since June 2015, the paediatric consultants have become concerned about a higher than usual number of neonatal deaths on the unit”.
What information does the Trust hold relating to June, July or August 2015 communications from the Paediatric Consultants expressing these concerns? Please include Directorate reports or minutes, emails from these Consultants or their secretaries, or other communications.
