Near miss incident on level crossing at Blaydon on 7th

Dear Network Rail Limited,

On 7th April 2017 at approximately 1500 BST, the 1454 Newcastle to Hexham, operated by a Class 142 Pacer led by coach 55783, was involved in a near collision with a cyclist crossing the line somewhere in the vicinity of Blaydon. Two cyclists were intending on crossing the line; one crossed in the immediate path of the train whilst the other waited for the train to pass first, resulting in the train driver sounding the horn continuously for 10+ seconds.

Please tell me the precise location and time of the incident, and supply copies of the resultant reports and investigations.
