Good afternoon,

I am writing to request information under FOISA.

How many Do Not Resuscitate (DNR/DNAR/DNACPR) orders have been placed on Forensic Mental Health patients within your health board since January 2019?

I would like this information broken down by:

Month (January 2019 - June 2020)Level of secure service (ie; high, medium low,
community)
Institution
Age of patient (over/under 40)
Gender of patient (male/female/non binary)
Race/ethnicity of Patient.

If aspects of this information are not available, then please provide what is, with a short explanation why data is unavailable. 

If possible, i would prefer the information in excel or csv format.

If you have any questions or require clarity on any of the above, please let me know as soon as possible.
