I know for certain some meetings were held in Bristol and Trowbridge and involved a significant number of agencies. I ask you to supply me with all agendas, notes and minutes and records you hold regarding these meetings please to include correspondence between your CCG and the AWP MHP NHS T relating to safeguarding meetings or issues raised or concerns ,that you hold for the periods between 01 01 2010 and todays date please.

Thank you so much for your very prompt response to my email sent via the whatdotheyknow website.
As a long suffering patient of AWP I have made a number of FOIA requests in a similar vein but requesting information about different matters and time periods.
You will see I made two different, yet similar , requests to SGCCG on 22 May 2014.
1. " The Falsification/Alteration of patients records at Avon & Wiltshire Mental Health Partnership - the chair of AWP now stating ' .. these Allegations have proved to be Incorrect ' " that did not receive any response from your CCG , and that was why I emailed you yesterday.
the second was:
2. " Multi Agency Safeguarding Meetings about the Avon and Wiltshire Mental Health Partnership NHS Trust " that does not now have any response from your CCG shown under that request.
Did NHS SGCCG provide a response under that request ?

Did NHS SGCCCG make any request to whatdotheyknow website to have your response removed - or are you aware of any other organisation that did ?

I note although my request should have been dealt with 'applicant blind' , within the response reference 1415/0038/SGCCG dated 17 July 2014 signed by Jane Gibbs your Chief Officer that you have just sent me, it states " I appreciate that you may have also obtained this from other organisations if it has been requested . " and yes I did REQUEST similar information from other NHS bodies closely involved in these matters - yet received different information from each one - but not a complete disclosure yet ?
That was why I approached the different NHS bodies involved in these matters - some are more transparent than others ??

By way of FOIA Advice and Assistance, where Jane Gibbs kindly tells me " a great deal of information was archived and is no longer held by the CCG ", I would like to know all the full FOIA contact details please for the organisation where this information is archived along with any reference numbers or details you hold that will help them to identify it.

My reason for persisting with this is very important to me - and has resulted in AWP delaying the transfer of my care for THREE AND A HALF YEARS , without any treatment or support being arranged for me in the meantime, despite my repeatedly begging for help. I am fighting to get my life back. .As I have well documented evidence of so many of my own sensitive and personal records prepared by them have been falsified, spun and removed by them, and I wish the Trust that is going to treat me to only receive ACCURATE & COMPLETE records so they will know the precise injuries and abuse I have suffered.
I am not alone in reporting such matters to their Board -as I have heard of another carer and another patient suffering similar falsification of their records - but nothing gets done.

Getting back to the response letter you sent of 17 July 2014, my TWO requests to your CCG are not accurately reflected in the section shown in BOLD within the letter - I believe that wording was included in a request sent elsewhere - but not to yourselves ?

I am left with the impression that archived reports and information relating to the recent history of AWPs falsification and/or Alteration has been archived ( buried ? ) , and adequate inspections are not now being carried out resulting in patients suffering harm and delay and a carer record being falsified and altered in respect of many VERY serious matters - the Board of AWP are ignoring people who report these matters to them at Board meetings - it is well documented.

I note in Jane Gibbs letter it also states " .. have been able to source some information relevant to your request and we are happy to provide this for you " .
Under your duty to confirm or deny, would you please very clearly inform me if you hold ANY further documents in respect of the TWO requests I actually made - disregarding the bold, highlighted numbered summary your CCG prepared - but relying on the words used in my actual request please ?

Please thank Jane Gibbs for her extremely kind offer in her third from final paragraph, and although I do not seek to go into protracted correspondence with your CCG - I am certain you will understand my concerns outlined within this email - and naturally IF you are still keeping a PROPER eye on AWP and wish to see evidence of the matters ive mentioned here - and much more ,you can write to my home address - available from CSCSU , AWP and NHS Wiltshire CCG.

Maybe due to my ' difficulties ', may I take you up on your kind offer to answer some queries, and provide clarifications and answer my concerns regarding the SGCCG handling of my three requests dated 22 May 2014.

It confused me where only one response was supplied on 17 07 2014 to the THREE requests I submitted on 22 May 2014.

The 17 short documents supplied under ' Documents1 17.zip ' are individually numbered ' Documents 1 ' to 17 . To keep it easy for you, and to help me understand will you please under each of the THREE requests identify the number of the document that relates to that request.

My i also ask you to break down the paragraphs of your letters of response provided so I can hopefully understand which parts of your letters reply to each of my requests ?

Where you state " More specifically, information relating to multi agency Adult Safeguarding meetings involving AWP held by South Glos CCG ,..." I wish to clarify - due to the shortage of qualifying documents you have so far supplied - that you staff have not confused the ' held ' element of your statement as meaning meetings held by your CCG, rather than information held by your CCG ?

Likewise " Information held on any Safeguarding meetings with Wiltshire Council relating to the above " stated could be confusing , I in fact asked about information relating to all adult safeguarding meetings about AWP - not quite the same.

You state you have been able to source ' some information ', although from the dates on the documents and the content, I would expect you to be holding considerably more information that I am entitled to see?
Please confirm or deny if this is the case ?
I have asked for information from you regarding where certain information of interest to me may be archived - hence I ask you to look again at my detailed correspondence with you and provide clear answers to the questions I have asked please under each request.

You state " As your request is complex in nature ... ", in order to assist you and to ensure you supplied all the information I am seeeking - that was why I split my request into three very clearly defined requests so no confusion would occurr , and maybe your CCG could learn that by amalgamating all three requests into one response it has caused confusion and probably not all the information I clearly asked for has been provided ?

I think I told you I am elderly and disabled, and still have an untreated mental health injury - 10 months after making these requests, I therefore ask you again to make a 'reasonable adjustment' under DDA and Equality Legislation please , and provide proper clear and complete responses under EACH of my requests - that are most certainly likely to now include many more documents than those you have so far provided if you take time to read my requests properly, and provide the information I am asking for please.
