Dear Sir or Madam,

I would like to know

1) on what date the Local Government Ombudsmen started to factor in the likely cost of an investigation into their decision whether to investigate a complaint or not? a copy of any internal documents on this issue prepared by each office would be appreciated (the CLAE is aware that the practice has been identified at the York Office);

2) the date the CLAE was made aware and factored the above issue into both its strategic and operational risk registers;

3) the date the internal auditors were made aware, and copies of all supporting documentation;

4) the date the CLAE audit committee were made aware, and copies of all documents including any advice that cost could be used as a factor not to investigate a complaint - as identified in any statement of assurance;

5) the date the CLAE accounting officer signed the CLAE accounts in the knowledge that complainants were being excluded from investigations into maladministration by Council's on cost grounds
