Dear Mr S Carroll,

Please find attached a final letter and data regarding your recent Freedom
of Information request.

We would also value your feedback regarding the way in which your request
was handled. You can provide us with direct feedback on our website at the
following web link:

[1]Information Governance Survey

Any feedback you provide will be strictly anonymous and much appreciated.

Summary of Response

Please be aware that I have decided not to release full details of all
staff/beneficiaries/applicants as this information falls under the
exemption in Section 40 subsections 2 and 3(A)(a) of the Freedom of
Information Act. Information withheld includes names of attendees of
meetings from both the NHSBSA and DHSC. Other redactions within the
minutes relate to discussions relating to individual cases. Although
beneficiaries/applicants aren’t referred by name, they could potentially
be identified if you knew the details of someone’s personal circumstances.

This is because it would breach the first data protection principle as:

a)  It is not fair to disclose these people’s personal details to the
world and is likely to cause damage or distress to
staff/beneficiaries/applicants.

b) These details are not of sufficient interest to the public to warrant
an intrusion into the privacy of those staff/beneficiaries/applicants.

Annex A attached sets out the exemption in full.

Remaining Information

A copy of the information is attached.

Dear Mr Carroll

Further to the below response, it has come to my attention that we’ve
issued the documents in unredacted form. Can I please ask that you delete
the documents from any inboxes and, if you are able to, from the what do
they know page. We have contacted what do they know directly requesting
their urgent assistance in this matter but if you are able to help by
deleting the documents which were issued in error, then please do so.
Please accept my sincerest apologies for this oversight.

Please find attached the correct documents with the redactions applied.

Chris
