Dear Mr Malone

Following your original request (copy of email included below), please now
find attached the documentation relevant to your request. Please note that
any sensitive information, names of children, confidential matters and
other exemptions from disclosure have been redacted from the
documentation.

Attached you will find:

o Minutes of all full governing body meetings from the point Mr Scott
Burt was elected Chair of Governors.
o Minutes of all governor committee meetings from the point Mr Scott
Burt was elected Chair of Governors.
o Copies of school held risk assessments.
o A summary of incidents involving Sam Malone as recorded within school
records. This includes a copy of the RIDDOR report from the incident
in October 2016. All documentation relating to the incident was shared
with you following the incident and as part of the subsequent
investigation carried out by an external party (the Local Authority)
with the investigation having now closed having confirmed that the
school was compliant and acted appropriately. As the investigation was
carried out by the Local Authority the school does not have a copy of
the full report to share with you however I am aware that the Local
Authority did report back to you at the conclusion of their
investigation. Should you require further clarity on the outcomes of
the investigation you will need to contact the Local Authority.
o Scanned contents of Sam Malone’s school office based personal file.
o With regards to the duty rota, duty rota staff are confirmed on a
daily basis as part of our staff briefing. This enables the school to
ensure that play times and lunch times are fully staffed as required
and can take into account staff absence to ensure a full complement of
staff are on duty each day. As such we do not update the rota on a
daily basis this is confirmed each day, as is the case in other
schools. As such a hardcopy of the duty rota is not supplied. Staffing
ratios were also discussed as part of the Local Authority
investigation and the systems and processes in school were assessed as
compliant and appropriate.

We believe that the attachments fulfil the obligations of your request
however if you believe this not to be the case please let me know and we
will respond.

Kind regards
