Please provide the audit trail covering the decision to publish the Chief Nursing Officer Letter of May 2009 regarding single sex accommodation as follows:

Specifically the date and name of the Minister who agreed the policy change set out in Annex E.
If no record of a Ministerial decision can be found (either in meeting notes or in a written response to a submission) please provide any record of a decision by any Senior Civil Servant (with a focus on the two signatories David Flory and Christine Beasley).

Please name the minister who agreed the policy change set out in Annex E of the CNO letter (assuming sign off was between 1st March 2009 and 7th May 2009 inclusive). Or confim if no record of a Ministerial decision can be found.
