• Referral source – number, locality, self, professional etc.
• Presenting issue, primary reason / diagnosis
• Appointments and discharges – attendances, activities, DNA’s and
completions
• Delivery of training
• Number of service users on each programme
• Number of exclusion referrals
• Number of attendance / DNA / completed courses
• Length of waiting lists and the reason, along with a remedial action
plan if required

• User feedback
• Service uses accessing health improvement services as a result of
support
• Number of people having screening / vaccinations/ health checks as a
