Medicine‑related patient safety incidents .
Dear Health and Social Care Board for Northern Ireland,
1. Can you please advise me for all NI trusts of the protocol in place to identify, report and record errors in medication management.
Which system is this posted to.
2. Can you advise me how accidental medication errors are clinically managed?
3. How are Adverse incidents caused by removing a prescribed medication recorded?
On which system are these recorded?
4. Do you recommend the nice guidelines below and thus should trusts use the "fair blame" model?

5. Do you recommend that all trusts record "mistakes" caused to patients in acute hospitals with reference to mal-administration of medication in hospitals.
On which system are these recorded.
6. When trusts become aware of adverse incidents and harm caused to patients, should there be a pathway where the patient and family be informed?
Is the guidance in line with national patient safety reporting systems.
