Making a request for a 999 call recording
Dear Yorkshire Ambulance Service NHS Trust,
I am making a request for a 999 call recording that was made on the 10th July 2014 by my partner. I went into labour at home and he delivered our baby with the fantastic help of your operator. Please could you help me out with this as I'd love to get it on a disc and put it in my sons memory box. Thank you so much.

The department now works remotely away from ambulance headquarters and the
pre-existing telephony is no longer in place. As such, the department
works exclusively by e-mail and your e-mail shall be actioned in the
normal way.
Additionally, if you usually send post into the department, please be
aware that there may be a delay in this post reaching the relevant staff
so if you are in a position to e-mail any documentation instead of via
post, it would be greatly appreciated.
Please ensure all contact is made via this e-mail inbox – the inbox will
be monitored daily between the hours of 0800hrs and 1600hrs Monday to
Friday excluding bank holidays.

Please find attached a letter explaining what is needed in order for us to process your request – Proof of ID and address.
If you would like the call to be unredacted in regard to your partner and any information they provide about themselves (name etc.) then if they could provide proof of address and photo ID we can take that as their consent for their data to be passed to you.
I have also attached the trusts Audio declaration form, this will need completing and returning (signed by both of you if you would like all parties data left in), before any call recording can be released to you.

Could I point out that you have submitted your request via WDTK.com. This is not a suitable method to submit such a request as all details within the email exchange with ourselves are uploaded to their website and placed in the public domain.
The process for submitting requests such as yours is via a Subject Access Request and not a Freedom of Information Request. We would strongly recommend that all future correspondence with the Trust regarding your Subject Access Request is from your personal email.
