Local medicine formulaires
Dear Hywel Dda Local Health Board,
With regards to the drug medicine formulary in the LHB could you please?
1. Confirm if the formulary is a joint primary and secondary care formulary?
2. What formulary is in place? Is the health board now live with inform formulary system? If not what is in place? Can this be accessed in primary and secondary care? Is there a web address whereby the public can view this list?
3. Please provide a copy of the diabetes chapter of the current formulary.
4. Provide any pathways/protocols for medicines in the use of diabetes.

to local medicine formularies.
Under the Act, the Health Board is required to supply the information to
you within 20 working days. In terms of your request, this means that the
information should be provided by 11 March 2013. If this is not possible,
a further letter will be sent advising you of the progress made in
satisfying your request.

managing requests for information that is covered under the Freedom of
