List of air quality management areas (AQMAs) in the UK

I understand that there are 600 air quality management areas (AQMAs) in the UK.

Please provide me with a list of these areas showing which local authorities they're in, their location and who is responsible for it, the date it was declared and other relevant information where possible.

I would prefer the information in electronic format, a CSV file if possible.
