I am writing to request the following information regarding the reception desk/office furniture that Glasgow Clyde College had procured and installed at the Langside Campus in October 2016.
1. Name of supplier of the said reception desk/office furniture provider
2. The design specification requested by the college.
3. The procurement exercise(s)
4. Please advise the decision maker for the procurement of the reception desk/Office equipment at the library at Langside Campus.
5. Evidence of ergonomic specifications including full Health & safety DSE report had been carried out before the purchase of said reception desk/office furniture, to ensure that it was suitable and fit for purpose.
6. The make and model of the reception desk/office furniture purchased
7. The Catalogue description of the reception desk/office furniture purchased
8. The total capital cost of the reception desk/office furniture purchased.
9. The Total installation cost of reception desk/office furniture purchased.
10. Details of any snagging from the installations.
11. Full detail of any maintenance/ongoing issues connected to this reception desk/office furniture within the Library at the Langside College.
12. Copies of all relevant invoices

Please provide a copy of the full procurement exercise for the purchase of this item via JCT SBCC Contract. Information not relevant to overall design specification for other items can and should be redacted.
Please provide a copy of any contract with JCT SBCC, Information not relevant to procurement and/or overall design specification for other items can and should be redacted.
Please include all evidence to support the claim that all works were carried out with the technical standards Scotland. relevant documentation on consultation with staff members, as to the product and design of the reception desk that ensures it was procured as fit for purpose and would cause no health and safety detriment to any member of staff or service user. Personal Information should be redacted.
Please also provide any reports from any consultation with staff that were considered before purchasing this item.
Please also provide evidence the staff consultation being considered before the purchase of this item.
Please also advise the roles of the college management team who authorised this purchase.
Please advise why there is not a recorded health and safety report/DSE report?
Please advise if there has been any DSE risk assessments since installation carried out installation and supply copies of such.
Please provide copies of all complaints from staff regarding this item. Personal information should be redacted.
Please advise if there has been any reported injuries or medical issues reported to the college regarding the use of this item, and whether you deem it fit for purpose?

This lack of recorded information in relation to this disclosure is rather disturbing, and I have concern that the principle's of good college governance in procurement have not been adhered too in this instance. To not record and hold very relevant information may constitute a serious lack of good governance by the college.

I am writing to request an internal review of Glasgow Clyde College's handling of my FOI request 'Library Reception Desk/Office Furniture Information'.
Q4 You stated that no recorded information was collated after the consultation with senior library staff members – I am dissatisfied with this response. Failure to record information from a serious consultation may be deemed as a failure of good governance.
Q9. Please provide ANY recorded information such as emails that provides evidence that discussions took place between staff and estates. This does not have to be a formal note, although not producing a formal note may be deemed as a failure of good governance.
Q8. No DSE assessment has been carried out. This is a failure of good governance, and requires to be reported to the HSE.
Q9. I am dissatisfied with your response relating to any complaints by staff regarding this desk. Please now provide any written record of any complaint whether formal or not. I did not stipulate whether any complaint was formal or informal – the term was generic.
Please note that under FOISA it is a criminal offence to attempt to conceal information held at the time of the request.
