On Wednesday 12 March, the Leng review team visited your trust.
Please provide all information, correspondence and documents you may hold in relation to this, such as:
1) Communication with the Leng review.
2) ALL communication with staff groups about the review and visit, including whether there were any differences in correspondence/announcement content, and date/time of sending to different staff groups (e.g. MAPs/resident doctors/consultants).
3) Any evidence you have submitted to the Leng review.
4) Any minutes/records you took from trust participation in the meetings.
5) Any other internal correspondence relating to the Leng review and this visit.

RE: 24FOI866 – Freedom of Information Request – Acknowledgement and
Section 8 compliance clarification
Please find attached our acknowledgement of your Freedom of Information
Act Request. However, before the Trust receives and processes your
request, can you please confirm that you are only submitting this request
for your personal use or that of an unstructured group and that you are
using your real name?

In essence, if your request is on behalf of an employer or someone else it
should be declared.
Please note: The Trust cannot assess fair processing without your response
to this email. We will not receive or process your request without
clarification. We will not send you a reminder letter and the Trust will
close your case after two months. Additionally, there is no right of
appeal with the ICO, if the real name of any organisation you work for is
not used.
If at any point in the process you no longer require any of the
information you have requested, we would be grateful if you would let us
know.
