Dear Leeds and York Partnership NHS Foundation Trust,

dear sirs

under foi can you confirm you have never been asked to jointly investigate the abuse of a current patient abused by third party partners of leeds cc

it is of great concern leeds cc are asked to investigate their own behaviour when it comes to abuse and it is great concern the costs to the tax payer of long term mental health treatment due to the behaviour of partners of leedscc

given the abuse if of a nature linked to negligence by leeds cc who at the nhs overseas any investigation into said act.please confirm who has a statutory responsability with regards to the duty of care for someone under a leeds post code

having completed the farce of a investigation by themselves i would like to know why the abuse of a customer of leeds is not investigated by anyone within social or adult care or the nhs mental health teams given the seriousness of the claim .it seems ridiculous a person caused harm by aprtners of leeds cc is asked to complain to leeds cc who then in turn ask the same team guilty of the offence to investigate themselves

who are nhs trust/leeds cc jointly accountable to in this situation please

Yours faithfully,

steve perkins
