A.      Currently Allocated to a Social Worker (i.e. being assessed or
reviewed)

B.      Currently Receiving Services (e.g. home care or a direct payment)

C.      Both A and B

D.      Either A or B

E.       Something else?

If you are unhappy with the service you have received in relation to your
request and wish to make a complaint or request a review of our decision,
