Further to my request https://www.whatdotheyknow.com/request/l... I have noticed there are several pages missing from the report.
1. the title and inside (prefacing) pages.
2. pages 80, 81, 82 and 83

Please could you answer these questions under FOI Acts.

Were those pages missing from the copy of the report you have sent or was it foi operator error? If it was foi error please could you resend the report with those pages included. If they were missing from the original, it is important for my purpose that two different reports are not mixed up and please could you send a different complete report from beginning to end.

Is the copy you sent me the final report ?- it seems so full of errors that it may just be a draft report. eg page numbering incorrect- no page 95 and many typographical errors.

I presume the copy of the report you sent me is of a photocopy, which I deduce from the misaligned pages. Is this correct? Is there any reason I was not sent a direct copy of an original?

Please could you tell me if the report was published and if so when?
How many copies of the report does the Council hold?
The Reports author, which I believe to be Lawson, does not seem to be mentioned in the report. How can this be? Am I correct in the author? Who was the author - what was his/her full name , and position at the time of writing the report. Who did he/she work for? Was he a Lewisham employee?
Are there any signed copies of the report? Under what legal framework was the Inquiry carried out?

Documents requested under FOI are usually photocopied and sent.
 
We did not release the original as we believe there are limited copies in
the Council and we would not give them away.

explain the delay in retrieving the additional information you have
requested.
 
To retrieve the requested document requires a manual search of our
archives. Arrangement to access the archives is by prior arrangement and
we are currently waiting for an appointment. Staff have been on leave and
unfortunately the area where the records are held in the basement was
recently flooded which has caused further delay.
 
After receiving a further email from you on 15^th October, we once again
contacted the team dealing with your request to ask for an update. We were
informed that they had still not been granted access the basement area. We
should have given you an update at this point but were hopeful that the
information was going to be retrieved swiftly.
 
We have now been told that the team will be given access to the basement
next week. Please be assured that we are dealing with your request as a
priority and endeavour to give you a full response in the next 7 days.

We now have the minutes from 2^nd July 1985. As we have read through them
it has become apparent that out of the 124 pages of minutes taken on that
date only 2 pages refer to Leeways children’s home. Would you just like
these 2 pages?
 
We can of course look at releasing the whole document but feel that it
would be irrelevant to your request.
 
Please note that any information that would be released is subject to any
exemptions that may apply.
