Latest LB of Newham Employee/Staff Survey Questions

I would like to submit the following Freedom of Information request:

Could the London Borough of Newham please provide a pdf or HTML file of all questions (full text) within the last London Borough of Newham employee/staff survey 2023 including the answers available for employees to select in relation to each question in the survey.

Please note this is not a request for any data provided by LBN staff for the survey but a request for the questions posed to employees/staff and available answers to select in the survey.

Thank you for response but to clarify, I am looking for ALL text presented to staff in the LBN staff survey 2023. Could I request confirmation that this is the only text (including any sub text) that was presented to LBN staff/employees.

If the London Borough of Newham confirm this is not the only questions/text presented to LBN employees when completing the survey, I am specifically looking for the question and sub text referencing the following phrases "BLM", "Black Lives Matter" or "George Floyd"

Please attach any LBN survey details in a PDF document in any subsequent response

The information provided to you in our response is the complete text of
the staff survey questions/sub text from the 2023 survey. 
 
We are not aware of any additional subtext that was included in the survey
that refers to 'BLM', 'Black Lives Matter' or 'George Floyd'.
