For those pressure ulcers that developed in the care of Central London Community Health (CLCH) a Root Cause Analysis (RCA) investigation is undertaken into the incident within the timeline set by the Patient Safety Team.

Please can you inform me:

1) how many category 3 and 4 pressure ulcers were reported by CLCH in the form of a serious incident in connection with Lansdowne Care Home.

2) How may RCAs were undertaken and the outcome.

Please provide the figures for years 2014, 2015 and 2016 year to date.

such, incidents in relation to Lansdowne Care Home would not be reported
by CLCH NHS Trust. We would therefore be grateful if you could clarify
whether your request relates to category 3 and 4 pressure ulcers reported
by CLCH, or category 3 and 4 pressure ulcers reported by Lansdowne Care
Home, or both (or indeed something else)?

 

Further, it would be helpful if you could clarify whether your second
question is intended as a standalone query (i.e. how many Root Cause
Analysis investigations were undertaken in 2014, 2015 and 2016 to date,
regardless of associated organisation/incident type), or whether it is
intended to follow on from your first question (i.e. of the incidents
reported in question one, how many RCAs were undertaken).
