IT Outage circa 26th March 2018
Dear Department for Work and Pensions,
I'm writing to you with regards to the 'IT Outage' that was reported by the Mirror on the 26th March 2018. I'd like the Department for Work and Pensions to answer the following questions:
Q1. When was the first incident relating to the impact first reported through your IT incident management software ('ServiceNow')?
Q2. Was there a scheduled change that caused this outage?
Q3. If yes to 'Q2', can you confirm against the scale used:
Q3.1. What business impact this change was given?
Q3.2. What priority this change was given?
Q3.3. What overall risk this change was given?
Q4. What was the estimated amount of impacted claimants who would not receive a payment on-time due to this incident?
Q5. What was the actual amount of impacted claimants who did not receive a payment on-time due to this incident?
Q6. What was the overall elapsed time to resolve the parent incident?
Q7. If yes to 'Q2', from what organisations were the resources involved in the change from and what role did they serve? (e.g. BT - Networks)
Q8. Has the department and/or its suppliers introduced any service improvements to prevent this kind of outage occuring on this scale again?
Q9. If yes to 'Q8', please can you provide any information relating to this improvement?
Q10. If no to 'Q8', please can you provide the rationale?
Q11. What is the estimated or actual loss made by the Department for Work and Pensions in working days due to this outage as well as the cost this has incurred?
