Isotretinoin Treatment for Acne Vulgaris

Dear Hampshire Hospitals NHS Foundation Trust,

Please provide me with the following information regarding the use of the drug isotretinoin (Roaccutane) as a treatment for acne vulgaris within the trust:

1. The number of patients prescribed isotretinoin at your hospitals for each month between January 2014 to January 2017 or yearly figures if monthly data is unavailable.

2. The total number of prescriptions for isotretinoin for each month of the same time period

(If information about prescriptions is not held, please give information about medication issued from your formulary instead)

3. Whether doctors prescribing isotretinoin are required to have undertaken training:
a. Specifically about isotretinoin
b. About effectively communicating information about risks involved to patients.

4. Details of any other specific requirements of doctors:
a. Prescribing isotretinoin
b. Reviewing patients taking isotretinoin (who may be part of a consultant's team but not a consultant themselves)

5. Whether there is a system or policy in place to ensure accurate and complete reporting of adverse drug reactions (ADRs) to the MHRA and if so, details of the nature of this system or policy.

6. Details of the trust's policy/procedure in relation to isotretinoin treatment, specifically regarding:
a. Whether assessment of patients' mental health history prior to treatment is conducted and how this is conducted and/or recorded.
b. Whether patients' mental health or mood state is assessed during treatment and how this is conducted
c. Whether this mental health information (points a and b) is recorded and the form this recording takes
d. How patients are counselled with information regarding risks of adverse effects
e. Exclusion criteria for isotretinoin treatment

7. Copies of local policy regarding the use of isotretinoin for acne vulgaris and details of any national advice followed (such as from the British Association of Dermatologists or the Summary of Product Characteristics)

8. A copy of any patient consent forms required before a patient commences isotretinoin treatment.

9. Whether the Trust is aware of patient(s) under the care of dermatology team(s), during or shortly after isotretinoin treatment, who have:
a. become clinically depressed or developed clinically relevant mood changes
b. attempted suicide
c. successfully committed suicide
